Liability Waiver *

Opportunity Barks LLC will endeavor to offer only sound, safe, and responsible care for my dog(s). However, I have been told and understand the risks inherent in dropping off my dog for daycare or boarding, including but not limited to interactions with other dogs and potential exposure to disease and parasites such as, but not limited to, fleas. Further, I am and will remain responsible for the actions of my dog at all times and I hereby agree to indemnify and hold harmless Opportunity Barks LLC of any and all claims of injury, expense, costs, or damages caused by the actions of my dog while under Opportunity Barks LLC’s care. I have been told by Opportunity Barks LLC and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others. I recognize that Opportunity Barks LLC is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

Emergency Care Permission *

Emergency Care Permission: I the undersigned, give Opportunity Barks permission to take my dog(s) to the nearest Animal Hospital for care/treatment in the case of emergency. This facility has my permission to administer any care/treatment deemed necessary. I will be solely responsible for all payments, in full, incurred at this facility. I also understand and agree that severe emergencies may require treatment before we are contacted.

Rough Housing *

I consent to bring my dog to Opportunity Barks even though I'm aware of the following information: there is inherent rough housing in dog play. All dogs under Opportunity Barks' care are screened for aggression, but fights can still occur. It is also possible for a puncture wound to happen during play sessions. In the event of a fight or injury, if needed we will contact you to bring your dog to the vet or we will bring them and bill you for the fees. We always attempt to get in touch with the owner or emergency contact before providing any medical attention.

Probation Policy *

All dogs accepted to board are considered “on probation” because behavior can change. If at anytime a dog’s behavior becomes unsuitable for this environment, the dog may not be able to return to board again. If it has been one year or more since a dog has boarded, they will need to be reevaluated before returning.

Reservations Policy *

A credit card is required for all reservations. **Holiday Reservations: Reservations for boarding over the New Year’s Day, Easter, Memorial Day, July 4th, Labor Day, Thanksgiving, and December holidays require a 50% nonrefundable deposit at the time of booking. Reservations will not be confirmed until the deposit is received. If a cancellation is made within 7 calendar days prior to arrival date, a fee will be charged based on the length of the reservation: $50.00 for boarding stays of 7 days or less, $75.00 for boarding stays of 8 – 14 days, $100.00 for boarding stays of at least 15 days.**

Photo Release *

I hereby grant to Opportunity Barks LLC, the right to use, publish, and reproduce my pet’s name and pictures of my pet in film or electronic (video) form. This permission shall continue unless I revoke the permission in writing.
